The Brain Injury Alliance of New Jersey is a statewide non-profit provider of quality services, supports, education and advocacy for people with brain injuries, their families, and the general public. We have an immediate opening for a per-diem Family Support Care Coordinator to work in the Southern NJ counties of Burlington, Camden, Atlantic, Gloucester, Salem, Cumberland, and Cape May,

Key Responsibilities:

· Care coordination for individuals with brain injuries

· Carry a caseload of up to 20 families, as needed in Southern New Jersey

· Program outreach and promotion

Qualifications

Bachelor’s degree in social work or related field with three years of experience in case management. Experience working with people with brain injury highly desirable. Master of Social Work, Public Health, Public Administration, Education or other closely related field can substitute for experience. Applicants must have valid driver’s license. (EOE)
